PAYMENTS

Dr. Caccavale is not a participating provider of insurance panels; however, he will provide paperwork for the patient to submit for reimbursement from plans with out-of-network benefits. Medicare/Medicaid is not accepted.

What does this practice policy of not accepting insurance mean to you? There is no requirement for this practice to report your diagnosis or other personal information to an insurance carrier. Your private treatment is confidential, unless, of course, you are an imminent danger to yourself or others. Also, there are no limits set by an insurance carrier on frequency, duration or length of your treatment. These parameters of treatment are determined by your situation and needs. A reduced fee scale, when appropriate, will be discussed. Payment is expected at the time of service.
